Wellness and Client Run Surveys

Adult System of Care is requesting Wellness and Client Run providers to disseminate the Wellness Client Satisfaction Survey to gather client feedback on elements of Wellness services as well as to provide a sample snapshot of employment and housing for clients. This one-page anonymous survey will be implemented over a one month period twice a year.

To administer the surveys, please print copies directly from your computer. Please do not make photocopies.The surveys have been scanned into special software that will help us quickly tally results. Photocopying the surveys may result in tilting the image and can result in delays in reporting the results. Please note that we have updated the Wellness Surveys in English and Spanish so please be sure to use the updated versions in these languages and please print them as double-sided.

For Directly Operated and Legal Entity Wellness Providers, please distribute the survey to your clients and for Client Run Centers, please distribute the survey to all Unique/Unduplicated clients who visit your program during the survey period. The Survey Administration Period is in the table below.  Once complete, please gather your completed surveys and deliver them by mail or in person to:
