Consumer Feedback Forms and Guidelines
The Consumer Feedback Forms (CFF) are to be administered biannual basis, in accordance with the administration periods outlined below. At the end of each administration period, there should be one completed CFF for each of the consumers of the Wellness or Client-Run Center. Completed forms are to be submitted to Urmi Patel, PsyD at 550 S. Vermont Ave, ASOC 12th floor, Los Angeles CA 90020 within two weeks after the administration period.

There are currently two (2) versions available: English and Spanish. Additional languages will become available once the CFF is finalized and no additional changes are going to be made.
